Information & Applications
Public Health Application
Submit two copies of the application 15 days before the next Authority meeting for sewer service or improvements tied to public health projects. LSA evaluates the request and provides any conditions needed for approval. Timely filing helps place your project on the meeting agenda.
Public Facility Application
For sewer service or improvements to public facilities, file this application in duplicate at least 15 days before the next Authority meeting. LSA will review the request and communicate any conditions for approval. Use this timeline to ensure projects can be scheduled for board consideration.
Industrial Application
Request industrial sewer service or improvements and include project specifics plus the escrow fee (5% of project cost, $500 minimum). LSA reviews the submittal for compliance and issues any conditions prior to approval. Processing will not proceed without the required escrow and documentation.
Multi-Family Application
For townhomes, condos, and apartment developments seeking sewer service or improvements, submit project information with the required escrow. LSA evaluates the submittal and issues conditions or approvals as appropriate. This application helps coordinate allocation and connection requirements for multi-unit sites.
Commercial Application
Use this form to request sewer service or improvements for commercial properties and submit required project details and escrow. LSA reviews capacity and compliance and may issue conditions prior to authorization. Approval is contingent on satisfactory documentation and fee payment.
Residential Application
Apply for residential sewer service by providing property, owner, and contact details; allocation and billing follow the Authority’s Procedures and Allocation Manual. LSA performs a field investigation to confirm the parcel can connect without extraordinary measures before approval. Signature/date fields acknowledge the terms of service and any conditions set by LSA.
